Athena Patient Portal
The Athena patient portal can be used for new client registration, as well as to conveniently update your personal information, view your upcoming appointments, see your lab results, and pay your bill. For scheduling, please call our front desk at 563-207-8932.
New Client Forms
New clients may register in advance using the Athena patient portal or by completing the forms below. If you prefer to register in person, please arrive 20-minutes early the day of your appointment.
ROI, GFE, Superbill
Some of our providers are able to accept insurance. Please call the phone number on the back of your insurance card to fully understand your policy's coverage before scheduling. For out of network providers, cash payment is due at time of service. We can provide a superbill for you to self-submit to your insurance company for reimbursement. (Learn More)
Additional forms may need to be completed for some services. These can be completed in-person prior to your appointment, or you can use the forms provided here.
Some of our providers do offer telehealth appointments on a case-by-case basis. Prior to your first telehealth appointment, please review and sign the telehealth waiver. Direct links are available below for some of our providers. Otherwise, your provider will send you a link for your appointment.
